Ortho Warranty: 


Billing & Payment Policy – GSR Dental LLC

  • Accounts are due upon receipt of each monthly invoice.

  • Billing begins on the invoice issue date.

  • Payment terms:

    • New clients: Net 15

    • Existing clients: Net 25 (extended after three successful, timely payments).

  • Late payments incur a 6.0% monthly service charge (18% annual rate).

  • Accounts past due will be subject to C.O.D. status until fully paid.

  • All cases and items remain property of GSR Dental LLC until the account is settled.

  • A $30 fee will apply to returned checks.

  • Clients agree to pay all applicable attorney’s fees, court costs, collection fees, and any other expenses incurred in recovering past due balances or insufficient fund checks, as permitted by law.

No-Fault Remake Policy

GSR Dental LLC offers remakes or adjustments at no additional charge within the warranty period if the original appliance is returned.

  • Chairside adjustments and deliver appliance to patient will incur additional charges.

Limited Warranty & Liability

GSR Dental LLC warrants that all devices are made according to your specifications and approval for intended use.

  • No other warranties are provided, including merchantability or fitness for a particular purpose.

  • Devices failing under normal wear and tear may be repaired or replaced at no charge for material and workmanship, up to three months for Hawley retainers.

Refund Policy

  • Refunds are available within 25 days from the invoice date.

  • The original appliance must be returned for a refund to be processed.

Exclusions & Special Conditions

  • No warranty or refund applies if the appliance was chewed or eaten by an animal or returned catastrophically damaged (multiple broken pieces).

  • Patients should be advised to keep appliances away from pets, as they are commonly attracted to them.
